Nurse Care Specialist Job Summary

You will work collaboratively with the healthcare team to develop and implement individualized care plans, ensuring that patients receive the best possible care. Your expertise in nursing diagnosis, patient assessment, and care planning will be essential in identifying patient needs and developing effective care strategies. You will also be responsible for educating patients and families about their care, promoting health and wellness, and advocating for their needs. Key Responsibilities:
  • Provide ongoing assessment, planning, implementation, and evaluation of nursing care for a defined group of patients.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Identify patient needs and develop effective care strategies.
  • Educate patients and families about their care, promoting health and wellness.
  • Advocate for patient needs and preferences.

Requirements:
  • Associates degree in Nursing from an accredited program.
  • Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire.
  • Basic Life Support required at hire.
  • Excellent communication and interpersonal skills.

Preferred Qualifications:
  • Ambulatory care nursing experience.
  • Primary nursing or case management experience.
  • Bachelor of science in Nursing (BSN).
