Post-Acute Care Primary Care Specialist

At the Necnic Group, we are dedicated to delivering exceptional care in post-acute settings. As a Primary Care Specialist, you will play a vital role in managing patients with chronic conditions and coordinating their care with other healthcare practitioners.
Key Responsibilities:
- Provide primary care in a post-acute setting for short-term and long-term patients
- Develop and implement personalized plans of care for patients with complex medical needs
- Order and interpret lab results to inform treatment decisions
- Prescribe and monitor medications to ensure patient safety and optimal outcomes
- Consult with physicians and collaborate with other healthcare professionals to deliver comprehensive patient care
- Educate patients and their caregivers on self-management techniques and healthy lifestyle habits
Requirements:
- New graduates welcome to apply
- Minimum of 6 months of primary care experience required
- Active and unrestricted Washington NP or PA license required
- Active and unrestricted DEA certification required
Compensation and Benefits:
We offer a competitive salary range of $115,000 - $167,000, based on productivity and performance. Additionally, our benefits package includes:
