Job Overview

The Pediatric Navigation Specialist - Clinical Coordination and Advocacy is a pivotal role at Medicine Journal, ensuring seamless access to care for patients within our pediatric multi-disciplinary clinics. As a key member of our team, you will be responsible for managing and coordinating all aspects of patient care, providing medical guidance, and acting as a patient advocate to guide families through the treatment process.

Key Responsibilities
  • Develop and maintain relationships with multidisciplinary providers and primary care physicians to enhance education and increase access to care.
  • Serve as a liaison between patients, families, and healthcare networks to facilitate communication and coordination.
  • Educate patients and families on diagnosis, treatment options, insurance choices, scheduling appointments, and completing necessary paperwork.
  • Secure referrals into our pediatric multi-disciplinary clinics and remove barriers that may hinder the treatment process.
  • Connect patients and families with community resources to support their overall well-being.
  • Collaborate with the leadership team to collect data, track outcomes, and contribute to strategic planning processes.

Requirements and Qualifications

To succeed in this role, you will need:
  • A current licensure to practice nursing in the state of Tennessee and BLS certification.
  • At least 2 years of experience in a related field, preferably with outpatient referrals and pediatric experience.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with medical teams and interdepartmental stakeholders.
  • Demonstrated knowledge of evidence-based practices, theories, principles, and processes in pediatric care.
