Job Title: Long Term Care Pharmacist

Job Summary: We are seeking a skilled Long Term Care Pharmacist to join our team at A-Line Staffing Solutions. The ideal candidate will have a strong background in pharmacy practice, with experience in long term care settings.

Responsibilities:

  • Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication information.
  • Ensure that appropriate laboratory tests are ordered, evaluated, and discussed with the healthcare team as needed.
  • Collaborate with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
  • Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.

Requirements:

  • Bachelor's Degree required; PharmD preferred.
  • 1+ year of pharmacist experience, with long term care experience preferred.
  • Valid/active Indiana RPh license.
