Job Summary

We are looking for an experienced Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to lead our clinical teams in delivering exceptional patient care. As a Clinical Team Coordinator, you will be responsible for overseeing admissions, discharges, and transfers, as well as coordinating care with residents, families, and healthcare providers.

Key Responsibilities
  • Coordinate admissions, discharges, and transfers, ensuring seamless transitions of care
  • Lead clinical teams in delivering high-quality patient care, including assessments, planning, implementation, and evaluation
  • Collaborate with residents, families, and healthcare providers to ensure comprehensive care plans
  • Manage daily work processes, prioritizing tasks and allocating resources effectively
  • Document resident information accurately and efficiently
Requirements
  • Bachelor's Degree in Nursing or Social Work
  • Registered Nurse (RN) license preferred; LVN/LPN valid license acceptable
  • TWO years of clinical nursing experience, preferably in long-term care or gerontology
  • Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits
Working Conditions

The Clinical Team Coordinator position requires strong communication, organizational, and leadership skills. You must be able to work effectively in a fast-paced environment, prioritize multiple tasks, and maintain accurate records.

Salary and Benefits

The salary for this position is approximately $63,000-$75,000 per year, depending on experience. PACS Inc offers a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.
