We are seeking an experienced Geriatric Care Specialist to join our team at Optum. This role is responsible for delivering high-quality, patient-centered care to members in their homes, nursing facilities, and assisted living settings. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work independently with minimal supervision.

Job Summary:

The Geriatric Care Specialist will be responsible for providing comprehensive assessments, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless transitions of care. This role requires strong analytical and problem-solving skills, as well as the ability to navigate complex medical systems.

Responsibilities:
  • Conduct comprehensive assessments of members' physical and mental health needs
  • Develop and implement individualized care plans that address members' unique needs and goals
  • Coordinate with healthcare providers to ensure timely and effective care transitions
  • Monitor members' progress and make adjustments to care plans as needed
  • Collaborate with community resources and support services to ensure members receive comprehensive care
Requirements:

To be considered for this role, candidates must have:

  • A master's degree in nursing or a related field
  • Current national NP or PA certification
  • Active and unrestricted APC license in the state of New Hampshire
  • Current DEA licensure/prescriptive authority
  • Ability to lift 30 pounds and navigate stairs and various dwelling conditions
  • Availability to work 24 hours per month, with expectations to work 16 hours during off-hours
