About the Job
We are seeking a highly skilled Physical Therapist to join our team at Gables Search Group, Inc. in New York State.
Job Summary:
The successful candidate will provide physical therapy services to adult patients in their homes, following physician's orders.
Key Responsibilities:
- Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
- Prepare an assessment of the patient's condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
- Recommend modification of plan of care to physician, as needed.
- Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
- Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
- Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).
- Include and educate the patient and caregiver(s) regarding treatment plan and goals.
- Attend physical therapy staff meetings and in-service presentations.
- Participate in case conferences.
- Participate in the development and periodic revision of the physician's Plan of Treatment.
- Assure appropriate safety practices and maintenance of equipment.
- Other duties as assigned.
Requirements:
- Graduate of an accredited Physical Therapy program.
- Current, valid Physical Therapy license issued by NYS Department of Education.
- One year of practical experience as a licensed physical therapist.

Work Environment:
The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.
Salary and Benefits:
We offer a competitive salary range of $85,000 to $110,000 per year, depending on experience, plus excellent benefits package, including health insurance, 401K, paid time off, and career advancement opportunities.
