Responsibilities:
  • Provide physical therapy services to patients in their homes, under the supervision of a licensed Physical Therapist.
  • Conduct initial evaluations and develop treatment plans to help patients achieve their goals.
  • Perform skilled procedures as ordered by physicians and according to established plans of care.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Document patient progress and maintain accurate medical records.

Requirements:
  • Graduate from a two-year college-level program approved by the American Physical Therapy Association.
  • Currently licensed in the state of Texas.
  • Two years of experience in Home Health preferred.
  • Spanish speaking preferred.
