Job Description:

As a Care Management Coordinator, you will play a critical role in supporting Care Managers to deliver high-quality services to members. Your responsibilities will include:

  • Member Engagement: Establish rapport with members, gather necessary information for care planning, and assist in coordinating appointments and services.">
  • Care Coordination: Assist in coordinating appointments, services, and transportation for members, ensuring seamless communication and efficient care delivery.">
  • Data Management: Maintain accurate and up-to-date records of member interactions, appointments, and services provided.">
  • Team Collaboration: Work closely with Care Managers and other team members to ensure effective communication and collaboration.">
  • Resource Assistance: Provide information and assistance to members in accessing community resources and social support services.">
  • Supportive Role: Share personal experiences sensitively when appropriate, providing a relatable perspective for members.">
  • Documentation: Complete all required documentation accurately and in a timely manner.">
  • Training and Development: Participate in training programs to enhance skills and stay updated on best practices in care management.">

Required Skills and Qualifications:

To be successful in this role, you must possess the following skills and qualifications:

  • Minimum of a high school diploma or equivalent.">
  • Individuals must be at least 18 years old.">
  • Certified Peer Support Specialist.">
  • Previous experience in a healthcare or community support role preferred.">
  • Excellent interpersonal and communication skills.">
  • Ability to work collaboratively within a team and independently when necessary.">
  • Empathy and understanding of individuals with behavioral health conditions.">
