Responsibilities
  • Conduct initial assessments, OASIS, and recertifications to ensure accurate and comprehensive patient care
  • Develop and implement individualized plans of care in collaboration with the interdisciplinary team
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
  • Educate patients and families on disease management, self-care, and health promotion
Requirements
  • RN license in New York
  • Experience in home care as a RN, LPN, CNA, or HHA preferred but not required
