Job Title: Hospice Care Specialist

We are seeking a skilled Hospice Care Specialist to join our team at Hospice of the Piedmont Inc. This is a full-time position that requires a registered nurse with expertise in pain management, symptom control, and crisis intervention.

About the Role:

As a Hospice Care Specialist, you will provide support to patients and families in need of hospice care services. Your primary responsibilities will include phone triage, assessing patient needs, managing pain and symptoms, and coordinating care over the phone or through home visits. You will also work closely with the interdisciplinary team to provide caseload coverage during staff shortages or absences.

Key Responsibilities:

  • Perform telephone or video assessments of patients' needs and concerns
  • Manage patients' pain and symptoms; obtain changes in orders as needed and update the patient's Plan of Care and medication list accordingly
  • Provide guidance, support, and education to hospice patients and families over the phone
  • Assess patients' need for home visits and schedule visits with the appropriate clinician
  • Communicate updates and interventions with the Interdisciplinary Group (IDG)
  • Document all communication and/or interventions in the patient's electronic medical record


Requirements:

To be successful in this role, you must have a graduate degree from an accredited nursing program and a current Virginia Registered Nurse license. A minimum of three years of experience as a registered nurse within the last five years is also required. Additionally, you should possess expertise or highly knowledgeable in pain management, symptom control, crisis intervention, and family support. Strong critical thinking skills, excellent clinical judgment, and experience with electronic medical records are also essential.

PREFERRED QUALIFICATIONS:

At least one year of hospice, home health, or palliative care experience is preferred. Hospice certification and prior hospice triage experience are also beneficial. Knowledge of Medicare Hospice Conditions of Participation (COPs), federal, and state regulations is a plus.
