Senior Mental Health Transition Specialist

WakeMed, a leading healthcare organization, is seeking a highly skilled Senior Mental Health Transition Specialist to join our team. This critical role plays a vital part in providing transitional care services to patients with substance use or mental health disorders.
About the Role:
The Senior Mental Health Transition Specialist will be responsible for screening referrals, assessing patient suitability for transitional care services, and arranging outpatient services when necessary. In collaboration with Peer Support Specialists, this individual will facilitate safe and timely transitions from hospital care to community-based services.
Requirements and Qualifications:
To be successful in this role, candidates must possess 3 years of clinical experience in mental health or substance abuse/addiction. A Bachelor's Degree in Nursing or Master's Degree in Social Work or Counseling is required, with a Master's Degree in Nursing being preferred. Additionally, a Licensed Clinical Social Worker certification is mandatory, as well as maintainable vehicle operator insurability.
Compensation and Benefits:
We offer an estimated annual salary range of $75,000 - $95,000 based on location and experience. Our comprehensive benefits package includes medical, dental, and vision insurance, paid time off, and opportunities for professional growth and development.
Work Schedule:
