Job Title: Registered Nurse - Community Based Outpatient Clinic

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Care Manager at our Community Based Outpatient Clinic. As a key member of our healthcare team, you will provide exceptional patient care and coordinate services with the VA medical center and stakeholders.

Key Responsibilities:
  • Provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) and manage patient care.
  • Supervise support staff and lead PACT team huddles.
  • Work with Primary Care Physicians to ensure adequate staffing for quality care delivery and continuity of care between the VAMC and the clinic.
  • Maintain universal precautions and infection control practices.
  • Administer medications per facility protocols and perform cardiopulmonary resuscitation (CPR) and assist during respiratory and cardiac arrest procedures.
  • Document all pertinent patient information in their medical records and Veteran Health Information Systems and Technology Architecture (VISTA) to demonstrate quality care delivery and promote continuity of care.
Requirements:
  • Minimum of 2 years of experience in Medical Surgical Unit, Outpatient Facility, Primary Care, and/or Adult Health.
  • Current and unrestricted nursing license in the state in which the RN practices.
  • Current Cardiopulmonary Resuscitation (CPR)/Basic Life Support (BLS) certification from AHA or MTN.
  • Knowledge of professional nursing principles, procedures, and techniques.
  • Ability to assess patient condition, recognize adverse signs and symptoms, and react swiftly in emergency situations.
