Job SummaryWe are seeking a compassionate Registered Nurse (RN) to join our interdisciplinary team as a Hospice Palliative Care Specialist. As a key member of our team, you will provide high-quality patient care, educate patients and families about palliative care, and work collaboratively with other healthcare professionals to ensure seamless delivery of services.
Key Responsibilities- Care for hospice patients in their homes, assisted living facilities, nursing homes, hospitals, or medical facilities, adjusting your schedule based on their needs.
- Conduct regular visits, assessing patients' conditions, and developing individualized care plans to address their physical, emotional, and spiritual needs.
- Educate patients and their families about the benefits of palliative care, encouraging them to focus on comfort rather than curative treatments.
- Collaborate with the interdisciplinary team, including social workers, physicians, chaplains, and nurses, to provide comprehensive support for patients and families.
- Maintain accurate records, documenting patient interactions, assessments, and care plans in accordance with regulatory requirements.
To be successful in this role, you must possess:
- A current RN license and a minimum of two years of experience in hospice, home health, community health, or hospital settings.
- Excellent communication and interpersonal skills, enabling you to build trust with patients, families, and healthcare professionals.
- The ability to work autonomously, making informed decisions at the bedside while adhering to VITAS Healthcare's policies and procedures.
- A valid driver's license and reliable transportation with adequate insurance coverage.
We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, plus benefits including:
