About the Opportunity

We are seeking a highly skilled Registered Nurse to fill a challenging role as a Nurse Case Manager for our outpatient care team. This is a full-time position that requires excellent clinical skills, strong communication abilities, and the ability to work effectively in a fast-paced environment.

Salary and Benefits

The salary range for this position is $46.90 - $70.36 per hour, depending on experience and qualifications. Job Description

  • Develop and implement population management programs to improve patient outcomes and reduce healthcare costs.
  • Risk-stratify patients according to established protocols and collaborate with interdisciplinary teams to assess patient needs and develop effective care plans.
  • Work closely with patients and families to create personalized care plans that address their unique needs and goals.
  • Delegate and oversee the care management of lower-risk patients and routine chronic disease population management tasks to assigned caregivers.
  • Track and evaluate transitions of care, ensuring seamless handoffs between clinics, hospitals, and other healthcare settings.

Requirements

  • Associate Degree in Nursing required; Bachelor's or Master's degree in Nursing preferred.
  • Minimum 3 years of clinical nursing experience in an ambulatory or clinic setting.
  • Registered Nurse licensure in the state of practice required.
  • Basic Life Support certification required upon hire; Advanced Cardiac Life Support certification required within 6 months.
