Job Summary

We are seeking a highly skilled Palliative Care Specialist to join our team at Avera Medical Group in Yankton, South Dakota. As a Palliative Care Specialist, you will play a critical role in delivering compassionate care to patients and their families, addressing their emotional and social needs in a clinical setting.

Responsibilities

The successful candidate will be responsible for assessing patients facing chronic illness, providing therapeutic services, and facilitating appropriate community referrals. You will work collaboratively with our multidisciplinary team to develop comprehensive patient care plans that address the physical, emotional, and social needs of our patients.

Requirements

">

To be considered for this exciting opportunity, you must possess a Master's degree in Social Work from an accredited program and be licensed as a Social Worker in the State of South Dakota. A Qualified Mental Health Professional Credential is preferred.

Avera Medical Group offers a competitive salary range of $28.75-$43.50 per hour, depending on experience.
