Job Summary

This role involves providing high-quality patient care and coordinating services across various levels and sites of care. As a Nurse Coordinator, you will work closely with the interdisciplinary team to ensure seamless care transitions and optimal outcomes for patients.

Key Responsibilities
  • Coordinate care for assigned patients, ensuring timely and appropriate provision of services
  • Assess, plan, implement, and evaluate care based on age-specific components
  • Provide patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings
  • Collaborate with members of the interdisciplinary team to influence care outcomes
Requirements

To be successful in this role, you must have a strong foundation in nursing principles and practices, as well as excellent communication and interpersonal skills. You should also possess the ability to work effectively in a fast-paced environment and adapt to changing priorities.

A Bachelor's degree in Nursing (BSN) or higher is required, along with a valid nursing license. At least 2 years of experience in a similar role is preferred, although exceptional candidates with relevant experience may be considered.
