Job Summary

We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients, addressing their emotional and social needs, and helping them achieve optimal health outcomes.

Key Responsibilities
  • Evaluate and develop personalized plans of care for patients, taking into account their emotional and social needs.
  • Assess patients' social and emotional states, identifying areas where they may need additional support or resources.
  • Provide education and guidance to patients and their families on managing social and emotional responses to illness or injury.
  • Collaborate with healthcare teams to develop and implement effective care plans.
  • Supervise and evaluate the performance of Social Work Assistants, ensuring that all medical social services are provided in compliance with company and professional standards.
Requirements
  • Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
  • Knowledge of and ability to assist with discharge planning needs, and to obtain community resources.
  • Ability to read, write, and speak fluent English.
