About the Role

Delta-T Group, a nationwide provider of interim staffing referrals and workforce solutions, is seeking a skilled Social Services Coordinator to join their team in Washington, DC.

Key Responsibilities
  • Taking a leadership position in supporting program structure and all youth within the program
  • Demonstrating initiative in supporting and empowering the youth and families
  • Ensuring client files, records, monthly reviews, and Individual Service Plans are accurate, updated, and secure
  • Participating in live supervision with clients and their families
  • Meeting regularly with the Program Manager for supervision
  • Demonstrating engagement and leadership in all weekly staff meetings
  • Assisting clients with discharge and aftercare planning as they transition into their next step following the program
  • Providing the Program Manager with timely and accurate documentation of all services provided, including client progress reports and reports of problems that require immediate attention
  • Overseeing and reviewing client assessments, providing case management services, and overseeing the development of individual treatment plans for clients and families
Requirements
  • Bachelor's or Master's Degree in Social Work
  • 1-5 years of experience in the Human Services field, preferably working with adolescents and/or young adults
  • Expert knowledge of issues surrounding homelessness
  • Demonstrated ability to work with community leaders to raise funding for programs and skills to conceptualize and effectively lead complex projects
