Job Summary

The Lawndale Christian Health Center seeks a skilled Nurse Practitioner to provide high-quality, patient-centered care to our ambulatory patients. As a key member of our care team, you will work collaboratively with physicians and other healthcare providers to develop effective treatment plans and provide comprehensive medical care.

Key Responsibilities
  • Consult and work collaboratively with physicians and other healthcare providers to develop effective treatment plans.
  • Provide direction to other personnel and review medical records to ensure that each patient's care plan and medical needs are being met.
  • Obtain health history and perform physical exams to determine and execute diagnoses and treatment plans in accordance with established standards, procedures, and practices.
  • Requisition lab tests and x-rays, write prescriptions, and make referrals as necessary.
  • Delegate patient care responsibilities to other care team members as appropriate.
  • Provide follow-up care for continuity of patient and family care by exchanging information and cooperating with other community health/social service agencies, hospitals, and healthcare providers.
  • Counsel patients and/or significant others concerning physical and developmental well-being, defining each individual's needs and problems.
  • Identify community health resources and guide patients and/or significant others in their use.
  • Attend staff meetings and provider meetings, and maintain involvement with LCHC committees to ensure quality of care for patients.
Requirements
  • Graduate of an accredited Nurse Practitioner program; Master's Degree preferred.
  • Current licensure or permit as RN in Illinois. Nurse Practitioner certification or registry eligible required.
  • Eligible for Certificate of Fitness (prescriptive privileges) in Illinois required.
Working Conditions

The Nurse Practitioner will work in a fast-paced, dynamic environment, providing care to approximately 7-8 patients per 4-hour shift per day. The ideal candidate will be able to work independently and make clinical assessments, with a strong commitment to providing high-quality patient care.
