Lifepoint Health is a leading provider of acute inpatient rehabilitation services. We are seeking a Physical Therapy Assistant to join our team at Kindred Hospital - Baldwin Park, California.
Job OverviewAs a Physical Therapy Assistant, you will play a vital role in the rehabilitation process, working under the supervision of a Physical Therapist to implement patient care plans and provide quality, compassionate care in a secure setting.
About the Role- You will work with mentally and/or physically impaired patients to restore, reinforce, and enhance their performance.
- Communicate patient progress or problems to your supervisor and other team members.
- Document patient care in accordance with regulatory, licensing, payer, and accrediting requirements.
- Instruct patient's family or nursing staff in follow-through programs.
You will also be responsible for maintaining equipment and work areas in a safe and clean condition, adhering to the Company's Code of Business Conduct, Corporate Compliance Agreement, appropriate professional standards, and applicable state/federal laws.
What You Will Do- Work collaboratively with the rehabilitation team to develop and implement individualized patient care plans.
- Provide physical therapy interventions, including exercises, modalities, and activities designed to improve patient function and mobility.
- Assist with patient scheduling, documenting patient care, and communicating with healthcare professionals to ensure seamless coordination of care.
- Contribute to a positive work environment by promoting teamwork, open communication, and a culture of excellence.
- A graduate degree from an accredited Physical Therapist Assistant program.
- Current state licensure as a Physical Therapist Assistant or equivalent in the states where services are rendered.
- CPR certification when required.
