Aveanna Healthcare is seeking a skilled Physical Therapist to join our dynamic home health care team in Douglas, GA. As a Physical Therapist, you will play a vital role in helping patients recover from illness or injury in the comfort of their own homes.
Job Summary:We are looking for a highly motivated and compassionate Physical Therapist to provide high-quality physical therapy services to our patients. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work independently in a fast-paced environment.
Responsibilities:- Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function.
- Develop and implement individualized plans of care to achieve patient goals and outcomes.
- Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions as needed.
- Collaborate with healthcare team members, including physicians, nurses, and other therapists, to ensure comprehensive patient care.
- Document patient progress and outcomes in accordance with company policies and procedures.
- Master's degree in Physical Therapy from an accredited university/program.
- Current, unrestricted state license as a Physical Therapist in the state of Georgia.
- Valid CPR certification.
- Strong organization and communication skills.
- Attention to detail and ability to work independently.
