Nurse Care Specialist

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

Key Responsibilities:

  • Conduct comprehensive assessments of patients to identify their unique needs and develop effective care plans
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
  • Collaborate with our interdisciplinary team to coordinate care and ensure seamless transitions between healthcare settings
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal outcomes

Requirements:

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
