We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our Home Health Care team in Washington County, OR.
The successful candidate will provide annual clinical assessments to patients in the comfort of their homes, identifying and reducing health risks while coordinating follow-up care to improve overall well-being.
As part of our Home and Community Care team, you'll be an integral member of our integrated medical teams who are redefining healthcare by bringing high-end medical service, compassionate care, and innovative solutions to vulnerable patient populations.
This role offers a unique opportunity to achieve work-life balance, with flexible provider schedules available between 20-40 hours per week. You can choose your schedule across the 7-day workweek without worrying about on-call, weekends, or holidays.
Key Responsibilities:
- Conduct thorough in-home assessments, including past medical history evaluation, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing when necessary.
- Identify diagnoses for care management and active medical management in the furtherance of treatment.
- Formulate a list of current and past medical conditions using clinical knowledge and judgment, along with assessment findings.
- Communicate assessment results to inform members' primary care physicians about potential gaps in care.
- Recognize urgent and emergent situations and intervene appropriately.
- Educate members on disease processes, medications, and compliance.

Requirements:
- Active NP or PA license or ability to obtain by start date, unencumbered and free of any open/unresolved disciplinary actions.
- Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date (For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification; for PAs - Physician Assistant national certification through NCCPA).
- Ability to complete the physical demands of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and various dwelling conditions, as well as sitting, standing, and kneeling for physical assessments.
- Reliable transportation for completing home visit assessments.
Preferred Qualifications:
- 1+ years of clinical experience in their highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
- Ability to travel up to 5% within licensed states if requested for additional incentives (with notice, based on business needs).
$40,000 - $60,000 per year, depending on qualifications and location. This estimate is based on average salaries for similar positions in the area.
