Job Summary

As a Registered Nurse with Dartmouth Health, you will play a vital role in ensuring the general well-being of residents through the delivery of high-quality services in a manner that is consistent with the philosophy of assisted living and that complies with the New Hampshire Nurse Practice Act and Resident Bill of Rights.

Key Responsibilities:

Ensure the general well-being of residents through the delivery of high-quality services

Manage and supervise resident associates in the absence of the Nurse Manager

Perform patient care using an integrated approach that optimizes patient safety and reduces the likelihood of medical/healthcare errors

Formulate an individual plan of care, revise plan as indicated by the patient's response to treatment, and evaluate overall plan for effectiveness

Communicate clinical information appropriately to provide for an interdisciplinary approach to the patient's needs

Collaborate with all ancillary departments

Coordinate and supervise patient care, as necessary

Demonstrate an ability to organize and prioritize job function

Perform physical/psychosocial assessment to collect data: follows the written plan of care, standards, practice guidelines

Utilize assessment data to identify patient care needs, to include patient education and discharge planning

Recognize and report abnormal assessment data and incorporate findings into the plan of care: validates and revises expected outcomes based on assessment

Accurately and legibly document all patient care activities, including but not limited to, wound assessment, patient care teaching, demonstrated progress toward goals, medication administration, nursing flow sheets, and all assessment forms

Delegate tasks to the health care team members appropriately

Demonstrate unit-specific competencies

Assist in orientation of new staff. Provides ongoing training of staff in order to provide high-quality service

Promotes accuracy and participates in the process of monthly medication records

Demonstrates an understanding of the aging process

Participates in conducting and maintaining resident's assessments using the assessment tool for assisted living

Demonstrates basic knowledge regarding lifeline and PHB system, initiates problem-solving if needed

Requirements

Graduate of an accredited school of nursing, current New Hampshire LPN/RN license

Must possess a high degree of interpersonal relation skills and capability of relating to a variety of people and personalities

Considerable initiative, judgment, and leadership skills are also required

Must have the demonstrated ability to direct and manage personnel

Must possess strong organizational and time-management skills

Must be willing to learn

Current BCLS certification required within 30 days of hire

Physical Requirements

Must be able to lift, carry, stand, bend, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time

Must be able to walk approximately four miles daily

Must be able to lift/push up to 50 pounds
