About the Job
We are seeking a highly skilled LVN Health Coach to join our team at WellMed-Kiest Park in Dallas, TX.
About the Role
As an LVN Health Coach, you will play a critical role in supporting Disease Management/Chronic Care Program requirements for medical group/health plan members. You will act as an educator, resource, and advocate for members and their families to ensure a maximum level of independence.
Key Responsibilities
- Work with primary care physicians and clinic staff to identify patients with high-risk diagnoses such as CHF, IHD, COPD/asthma, and diabetes.
- Contact and perform initial interviews with patients who require health coaching programs.
- Conduct Chronic Care Model visits and review patient informal and formal support systems.
- Provide necessary coaching to reduce or eliminate high-risk behaviors.
- Identify required goals for each patient and advise feasible options for achieving them.
- Ensure patients are aware of health issues, concerns, and ways to combat them.
- Raise awareness about available exercises, weight loss programs, and dietary requirements for a healthy lifestyle.
- Utilize motivational interviewing techniques to assist patients in completing self-management goal/action plans.
- Document patient needs and outcomes in electronic medical records.
- Maintain current knowledge of chronic diseases, treatments, and complex medications.
- Assist with referrals, coordinate transitions of care, and establish trusting relationships with patients and staff.
Requirements
- High School diploma or GED
- Licensed Vocational Nurse with a current license to practice in the state of employment
- 2+ years of experience in a physician's office, clinical, or hospital setting
- Cardiac, medical-surgical, and/or critical care experience
- Experience related to patient education and/or motivational interviewing skills and self-management goal setting
- Proficient knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD
- Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook
- Proven excellent verbal and written skills
- Proven ability to interact productively with individuals and multidisciplinary teams
- Proven excellent organizational and prioritization skills
Preferred Qualifications
- 5+ years of experience in a physician's office, clinical, or hospital setting
- Proven knowledge of managed care, referral processes, claims, and ICD-9 and CPT coding
