Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team as a Healthcare Home Assessment Specialist in New York State. This is a challenging and rewarding role that requires strong clinical skills, excellent communication abilities, and a commitment to delivering high-quality patient care.

About the Role

In this position, you will perform initial in-home pre-evaluations of patients, assessing their health status, strengths, care needs, and preferences to guide long-term care service plans. You will also review medical documentation and health referral forms, enter evaluation data into an electronic form, and document any concerns or issues that arise during the evaluation process.

You will support continuity of care by coordinating with interdisciplinary care team members, discussing healthcare options with consumers, and collecting quality review data to support outcome measurements. Additionally, you will maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population.

Key Responsibilities
  • Perform initial in-home pre-evaluations of patients, assessing their health status, strengths, care needs, and preferences
  • Review medical documentation and health referral forms relevant to the case
  • Enter evaluation data into an electronic form and transmit as required
  • Document any concerns, conflicting information, or issues that arise during the evaluation process
  • Support continuity of care by coordinating with interdisciplinary care team members
  • Discuss healthcare options with consumers and collect quality review data to support outcome measurements
Requirements

To be successful in this role, you must have a minimum of two years of experience in a home setting within the last ten years, current RN licensure in New York State, and BLS certification. You should also be proficient in conducting comprehensive patient assessments, comfortable performing hybrid telephonic and in-field assessments, and able to sign a COI form upon start.

As a Healthcare Home Assessment Specialist, you will work independently and within a team to provide high-quality patient care. You will also have the opportunity to develop your skills and expertise in a dynamic and supportive environment.

Salary

The estimated annual salary for this position is $85,000 - $105,000 based on experience and location.
