We are seeking a dedicated and compassionate Physical Therapist to join our esteemed team at JAG Physical Therapy. As a leading provider of outpatient physical therapy services, we prioritize patient care and rehabilitation excellence.
About the Role:
The successful candidate will be responsible for assessing patients' conditions, creating personalized treatment plans, and delivering high-quality care in a supportive environment. Our ideal candidate possesses strong clinical skills, excellent communication abilities, and a commitment to fostering positive outcomes for our patients.
Key Responsibilities:
- Collect and record patient data, ensuring accurate documentation and compliance with regulatory requirements
- Develop and implement effective treatment plans, tailored to individual patient needs and goals
- Collaborate with healthcare professionals to ensure seamless care coordination and optimal patient outcomes
- Provide education and training to patients and their support systems, empowering them to achieve maximum independence and functionality
Requirements:
- Bachelor's or Master's degree in Physical Therapy (Doctorate degree preferred)
- Current licensure as a Physical Therapist in the state of practice
- Valid CPR certification
- Experience with EMR software (preferable)
- Strong clinical decision-making skills, effective communication, and teamwork abilities
