About the Role
Community Options, a national non-profit agency providing residential group home, day habilitation and supported employment services to individuals with disabilities, is seeking a skilled Licensed Practical Nurse (LPN) in River Edge, NJ. This role offers a unique opportunity to deliver care to individuals in a community setting while working collaboratively with health team members and significant others.

Key Responsibilities
  • Deliver high-quality healthcare procedures ordered by physicians, adhering to professional practice standards and laws.
  • Maintain accurate records of individual attendance at medical appointments per their Individual Support Plan (ISP).
  • Collaborate with supervisors to address concerns and issues related to nursing services provision.
  • Create comprehensive medical/health reports detailing general health, care goals, and physician recommendations.
  • Contribute to developing and updating Individual Support Plans (ISP)/Person Centered Plans (PCP) with interdisciplinary teams.
  • Train staff on the medical needs of assigned individuals.
  • Monitor and refer individuals for medical services as needed.
  • Ensure adherence to medication administration logs and maintain accessible health service supplies.
  • Prioritize program documentation and billable records accuracy and timeliness.
  • Foster support networks by building relationships with community members, businesses, and families.
  • Participate in community, social, and recreational activities with individuals.
  • Support personal futures planning through circles of support.
  • Implement strict OSHA procedures regarding blood-borne pathogens.

Requirements
  • Bachelor's degree or equivalent experience in the nursing field.
  • A minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid nursing license.
  • Valid driver's license with a satisfactory driving record.
  • Completion of state and agency required training per guidelines.
  • Ability to calculate figures related to medication dosages.
  • Understanding and commitment to community-based support for persons with disabilities.
  • Effective crisis management skills.
  • Strong relationship-building abilities with individuals receiving supports.
  • Proficiency in Microsoft Office.
  • Excellent communication and presentation skills.
  • Time management expertise.

Working Conditions
  • Work environments include offices, residential programs, day programs, and community settings.
  • May be exposed to changing weather conditions when working outside.
  • Flexible schedules may include evening and weekend hours due to business needs.
  • On-call requirements may arise in cooperation with other management staff.
  • Overtime opportunities may be available.
  • Frequent physical exertion may be necessary, including lifting and stretching.
  • Occasional assistance with wheelchair transfers may be required.
  • Transportation responsibilities may involve utilizing company-provided vehicles or personal vehicles.
  • Exposure to various medical conditions, communicable diseases, blood, body fluids, or tissues is possible.
