Primary Care Opportunities

The Department of Veteran Affairs is seeking skilled Family Practice or Internal Medicine trained physicians to join our Community Based Primary Care Clinic team in Peoria. As a Primary Care Physician Specialist, you will provide comprehensive care to a panel of patients under the Patient Aligned Care Team (PACT) model. Your responsibilities will include managing chronic conditions, evaluating and treating inter-current medical conditions, and performing disease prevention related activities.

Key Responsibilities

  • Collaborate with a Patient Aligned Care Team (PACT) to provide accessible, comprehensive, coordinated, continuous care to an assigned panel of patients.
  • Screen for, evaluate and treat uncomplicated common mental health disorders, including depression and common addictions, with referrals made to mental health providers for further evaluation and treatment as appropriate.
  • Provide clinical oversight and participate in peer review of other primary care providers through periodic chart reviews.
  • Act as a surrogate for absent primary care providers when needed, to include review of CPRS view alerts.
  • Regularly attend primary care staff meetings, departmental meetings and medical staff meetings.

Monday-Friday; 7:45 a.m. - 4:15 p.m.
