Job SummaryWe are seeking an experienced LPN with a passion for home health care to provide high-quality patient services on this picturesque island. As a Home Health LPN, you will be responsible for delivering compassionate and personalized care to patients in their homes, promoting independence and improving their overall quality of life.
Responsibilities- Provide skilled nursing care to patients in their homes, including wound management, medication administration, and vital sign monitoring
- Assess patients' needs and develop individualized care plans to meet their unique requirements
- Collaborate with other healthcare professionals to ensure seamless patient care and coordination
- Document patient progress and communicate effectively with patients, families, and healthcare teams
To be considered for this exciting opportunity, applicants must possess:
- A valid HI License
- At least 2 years of experience as a LPN
- Experience in home health care is required; experience in gerontology or hospice care is preferred
- Certifications in BLS (AHA)
