Job Summary

We're seeking a skilled Nurse Practitioner to join our team as a Traveling Advanced Practice Provider. As a key member of our geriatric care team, you'll provide primary, urgent, and acute medical care to patients in their homes, working collaboratively with our physicians, social workers, paramedics, and care team coordinators to deliver concierge-level care.

Responsibilities
  • Provide primary care to patients in their homes, addressing urgent care needs as needed
  • Collaborate with community teams to complete care assessments and deliver quality care while maintaining company metrics
  • Facilitate staff, patient, and family decision making by providing educational resources and counseling
  • Deliver palliative care and end-of-life care to patients and families, as needed
  • Participate in family meetings with Wellbe community teams to address complicated family dynamics and goals of care
Requirements
  • Master's Degree in Nursing
  • Valid nurse practitioner license in state of operations and current board certification in your field
  • 5+ years of progressive care experience
  • Valid driver's license, car insurance, and access to a car for patient visits
  • Expertise in diagnosis and treatment of chronic and urgent medical/psychological conditions of older adults
  • Experience in end-of-life, geriatrics, internal medicine, and palliative care
