Dietitian - Community Wellness East (Diabetes Education) - Days

We are seeking a highly skilled Registered Dietitian to join our Community Wellness East team as a Diabetes Education Specialist. This is a full-time position working Monday through Friday, 8:00am to 5:00pm, with occasional evening hours for class. The successful candidate will provide medical nutrition therapy (MNT) in an outpatient setting, assessing and interpreting patient data to develop and implement individualized nutrition care plans. The ideal candidate will have a strong background in diabetes education and experience working with patients with Type 1 and Type 2 diabetes, as well as gestational diabetes.
Key Responsibilities:
Develop and implement individualized nutrition care plans for patients with diabetes
Provide medical nutrition therapy (MNT) in an outpatient setting
Assess and interpret patient data to inform nutrition care plans
Collaborate with healthcare teams to ensure comprehensive patient care
Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
Bachelor's degree in Food and Nutrition or related field from a program approved by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
1 year of supervised practice experience or 1 year of on-the-job experience
Registered Dietitian/Registered Dietitian Nutritionist (RD/RDN) certification within 90 days of hire
Licensed Dietitian-Nutritionist (LDN) certification within 180 days of hire
