We are seeking an experienced Community Resource Specialist to join our team at AgeSpan, a leading provider of eldercare services.

Job Summary

The successful candidate will be responsible for providing information and referral services to individuals and families in need of support. This includes assessing client needs, identifying relevant resources, and connecting them with the right services.

Responsibilities
  • Provide information and referral services to clients through various channels, including phone, email, and in-person interactions
  • Assess client needs and identify relevant resources, such as home care services, adult day programs, and transportation options
  • Connect clients with the right services, ensuring a smooth transition and minimizing barriers to access
  • Maintain accurate records and documentation, adhering to regulatory requirements and agency policies
  • Promote awareness of AgeSpan's services and resources within the community, building partnerships and collaborations as needed
Requirements
  • Bachelor's degree in a related field, or equivalent experience in social work or a related field
  • Strong knowledge of eldercare services, resources, and regulations
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse stakeholders
  • Proficiency in Microsoft Office and web-based databases
  • Able to work independently and as part of a team, prioritizing tasks and managing multiple projects simultaneously
