Heart Failure Disease Specialty Coordinator

Establishes and maintains a disease-specific network with community and outlying hospitals, focusing on Vanderbilt University Medical Center Affiliate Hospitals. Ensures compliance with TJC disease-specific certification and accreditation requirements, collaborating with the Accreditation and Standards Department. Coordinates timely and optimal care for patients presenting with acute heart failure, utilizing evidence-based national guidelines and standards. Develops and implements disease-specific education programs and ensures follow-up care for all patients. Collects and analyzes outcomes data for the specified population, serving as the primary point of contact for patients upon discharge. Works closely with Patient Care Center leadership to achieve program goals.

- Relevant work experience: 3 years.
- Education: Bachelor's degree.
