Licensed Practical Nurse Inpatient Hospital Position

Responsibilities:
The Psychiatric/Mental Health LPN is responsible for collecting and comparing data to conduct a focused assessment of a patient's health status. They collaborate with providers and Registered Nurses to coordinate psychiatric services for patients experiencing psychiatric/mental health disorders in the clinic setting.
They provide care in a non-judgmental, non-discriminatory manner that is sensitive to the patient's developmental age and to the patient's and family's diversity, preserving their autonomy, dignity, and rights.
They perform appropriate interventions as ordered by physician in an accurate and timely manner, including needed testing such as lab draws, EKG, UDS, Genesight, and HcG testing.
They document results and provide safe handoffs to providers about the results.
They screen calls from patients and families, address the need, and/or transfer the calls to a member of the clinic nursing support team based on their scope of practice.
They track and communicate pertinent medical and psychiatric information both verbally and through written documentation in EHR.
They maintain confidentiality in matters related to patients and families.
They provide care management for patients in person and over the phone utilizing psychosocial, physical, and behavioral data gathering related to specific conditions and collaborate with RN/Provider to provide individualized directives based on this data.
They adhere to safe infection control practices, including good hand hygiene, standard precautions, cleaning equipment, and teach patients practices such as washing their hands and covering their cough.
They maintain a safe, secure, and healthy work environment by adhering to operating guidelines, policies, and procedures.
They complete medication refills in an accurate manner consistent with policy and good nursing practice, including verifying allergies and/or contraindications and monitoring for side effects.
They administer long-acting injections in a safe manner using safe nursing practices and consistent with clinic protocol. They educate patients and families about medications.
They obtain insurance pre-authorizations for medications and track prior authorizations.
They assist in completing and coordinating referrals.
They orient and educate in gender, cultural differences, and age-appropriate care.
They monitor and address requests received in the task management system, assign tasks to team members based on scope of practice.
They communicate effectively with interdisciplinary team members. They collaborate with the team, seek timely advice, and give and always accept feedback to ensure safe patient care.
Qualifications:
Graduate of an accredited school of nursing.
Current licensure as an LPN with the Minnesota State Board of Nursing is required.
Evidence of 1 year of psychiatric/mental health nursing experience within the past two years preferred.
Must have critical thinking skills and ability to problem solve.
Ability to communicate and make safe judgment calls in stressful situations.
Ability to intervene in pre-crisis or crisis situations by utilizing de-escalation techniques, including developing a plan of safety for patients as needed.
Ability to multi-task, use time efficiently, and prioritize appropriately.Ability to communicate in a professional manner in both verbal and written format and model a positive teamwork attitude.
Interpersonal skills, maintaining confidentiality, remaining open to others' ideas, and exhibiting willingness to try new things.
Must have basic computer skills using Microsoft Office products and EHR.
Ability to react quickly and calmly as part of a team to handle volatile and potentially emergent situations.
Looks for ways to improve and promote quality and demonstrates accuracy and thoroughness.
