What You Will Do
As a Registered Nurse Home Health PRN, you will contribute to our success by:
- Evaluating patient needs and developing individualized plans of care.
- Providing direct patient care, including assessments, planning, implementing, and evaluating the plan of treatment.
- Maintaining accurate and comprehensive clinical records, including documentation of patient progress and any changes to the plan of care.
- Communicating effectively with patients, families, and other healthcare team members to ensure seamless coordination of care.
- Participating in interdisciplinary team meetings to discuss patient progress and develop strategies for improvement.
What We Require
- A current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area, with a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.
- Associates Degree or Bachelor's Degree in Nursing is preferred, as well as previous Home Health or Hospice Experience.
