**Job Summary:**

We are seeking a compassionate and dedicated Caring Spiritual Care Coordinator to join our team at Anchor Health.

**About the Role:**

The successful candidate will be responsible for providing spiritual care services to patients and their families/caregivers, ensuring their spiritual needs are met in a timely and effective manner. This role involves coordinating with other spiritual counselors and healthcare professionals to develop individualized plans of care that address the unique spiritual needs of each patient.

Main Responsibilities:

  • Conduct spiritual assessments of patients and families/caregivers, identifying areas of need and developing strategies to address them
  • Collaborate with interdisciplinary teams to develop comprehensive care plans that incorporate spiritual care
  • Provide direct spiritual care to patients and families/caregivers, including facilitating worship services, counseling, and support groups
  • Serve as a liaison between community chaplains, spiritual counselors, and healthcare professionals to ensure seamless coordination of care
  • Maintain accurate records of spiritual care services provided, including documentation of progress and outcomes

Requirements:

To be successful in this role, you will need to possess a strong theological background, excellent communication and interpersonal skills, and the ability to work collaboratively as part of an interdisciplinary team. A graduate degree in theology or a related field is required, with relevant experience working in hospice care or a similar setting preferred.
