Clinical Care Coordinator

As a Clinical Care Coordinator at Brightli, you will play a vital role in delivering high-quality nursing services to clients in residential care facilities, behavioral health, and substance use disorder treatment programs.
About the Role
This position requires a licensed practical nurse (LPN) to provide nursing services, maintain a safe and therapeutic environment, and support clients through various nursing duties. The ideal candidate will have strong communication skills, be proficient in documenting medication administration and nursing assessments, and possess a willingness to learn and grow with our organization.
Key Responsibilities
- Administer medications to clients according to service line requirements.
- Perform nursing assessments, including physical and mental health assessments, and psychiatric screeners.
- Document medication administration and nursing assessments accurately.
- Communicate with clients regarding medication questions and needs.
- Room clients, verify client data and medical history, and obtain necessary physical preliminary requirements.
Requirements
- Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
- Graduation from an accredited practical nurse program.
