We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Our Lady of the Lake Regional Medical Center. As an LPN Care Coordinator, you will play a vital role in providing high-quality patient care and ensuring seamless coordination between healthcare professionals.

About the Role

In this role, you will work under the direction of a qualified professional nurse, participating in the planning, implementation, and evaluation of nursing care that aligns with the objectives, standards, and policies of the department. You will collaborate with members of the nursing and medical staffs as well as other hospital departments to ensure effective communication and coordination of care.

You will be responsible for performing COVID nasal swab tests and dressing changes when appropriate. Additionally, you will ensure that all necessary documentation is completed accurately and thoroughly.

Requirements
  • A current LPN license to practice in the state.
  • Graduate of an accredited LPN program.
  • Experience in a hospital acute care setting preferred.
  • BLS certification.

To succeed in this role, you must possess excellent oral and written communication skills, enabling you to effectively communicate with patients, families, and healthcare teams.
