About the Role

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Agape Care Group. In this role, you will play a vital part in delivering exceptional patient care and support to individuals facing life-limiting illnesses and their families.

As a Clinical Care Coordinator, you will be responsible for assessing patients' needs, providing guidance, and coordinating care with the care team, facility, and patients' caregivers. Your expertise and judgment will ensure that patients receive proper and timely care.

Key Responsibilities:

  • Assess patients' needs and provide guidance on care coordination
  • Work effectively with the care team, facility, and patients' caregivers to deliver seamless care
  • Ensure patients receive proper and timely care

What We Offer

We offer a competitive salary range of $85,000 - $110,000 per annum, depending on experience and location.
