Advanced Practice Provider: Community Health Mobile Program Specialist

Responsibilities:
- Provide primary care services to patients across the lifespan, including pediatric through geriatric populations.
- Offer telehealth services and chronic disease management to patients in their homes and community settings.
- Collaborate with our integrated team to coordinate care and provide comprehensive services to patients.
- Participate in community outreach and partnerships to expand access to care and improve health outcomes.
Requirements:
- Family Medicine certification and experience required.
- 1 year of APRN experience required.
- 2 years of APRN experience preferred.
