About the Role

We are seeking a highly skilled and experienced Senior Nurse Practitioner Care Manager to join our team at CenterLight Health System. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and leading our care management efforts.

Job Summary

The Senior Nurse Practitioner Care Manager will work closely with our community primary care physicians to ensure proper oversight and coordination of participant care. This role will involve working as a liaison between participants, community physicians, and our plan staff to develop and implement individualized plans of care.

Responsibilities
  • Provide direct patient care and coordinate services to meet the needs of our frail and elderly population.
  • Supervise and manage Nurse Practitioners assigned to your location, ensuring that they have the necessary support and resources to provide high-quality care.
  • Collaborate with other members of the interdisciplinary team to develop and implement comprehensive care plans.
  • Represent our organization in a professional manner, communicating effectively with participants, families, and healthcare providers.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
  • Maintain accurate and complete records of patient interactions and care provided.
Requirements
  • A minimum of two years of experience in clinical nursing practice in-home care, care management, or a related field.
  • Licensed to practice as a Nurse Practitioner in New York State.
  • Effective communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
  • High-level analytical and research skills, with the ability to interpret complex data and make informed decisions.
  • Basic computer skills in Microsoft Office.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
Benefits

We offer a competitive salary range of $155,000 to $165,000 per year, based on experience. Our benefits package includes medical, dental, and vision coverage, as well as a 401(k) plan and paid time off.
