About the Role: As a Registered Nurse Home Health, you will play a vital part in our mission to provide high-quality care and support to patients and their families. Salary and Benefits: The estimated salary for this position is $80,000 - $120,000 per annum, depending on experience. In addition to a competitive compensation package, benefits include work autonomy and flexible schedules, 1:1 patient care, comprehensive insurance plans, 401(K) with employer match, and paid time off.

Key Responsibilities:
  • Ensure that all activities performed align with the vision of Elara Caring's board of directors, executive team, and the leadership of the Home Health team.
  • Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
  • Assess assigned case load of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on-site evaluations.
  • Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.

Required Skills and Qualifications: To be successful in this role, you will need:
  • A current, unrestricted state RN license.
  • At least 1 year of experience in a clinical care setting as a nurse.
  • The ability and willingness to travel within branch/office coverage area.
