**Job Overview:**
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at the Scotts County Outpatient Clinic in Scottsburg, IN. As a key member of our Patient Aligned Care Team (PACT), you will play a vital role in providing exceptional patient care and supporting our mission to serve America's veterans.
**About the Role:**
This LPN position is located within Primary Care and requires a strong foundation in nursing principles, excellent communication skills, and the ability to work effectively in a fast-paced environment. You will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and contributing to a positive and inclusive work culture.
**Responsibilities:**
- Provide full range of practical nursing care to patients with various physical and behavioral problems
- Complete patient-focused assessments, recognizing risk factors and signs of withdrawal
- Administer medications and injections, observing and documenting patient response and reporting changes in condition
- Perform urine specimen collections, breathalyzer tests, and other point-of-care specimen collections as ordered
- Label and store lab specimens appropriately
- Participate in nursing reports and care team meetings to ensure continuity of care
- Implement patient/family teaching programs related to health promotion and reinforce instructions provided by RNs and physicians
- Initiate therapeutic communications with patients and families utilizing formal and informal communication processes
**Requirements:**
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the Accreditation Commission for Education in Nursing (ACEN)
- Full, active, current, and unrestricted licensure as a licensed practical nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States or District of Columbia
- Minimum 1 year of experience at the GS-5 level or equivalent, fully meeting all performance requirements
- Ability to observe, identify, and respond to patient needs for care, including medication, equipment-assisted care, and patient/family education
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and interdisciplinary teams
