Job Overview

We are seeking a highly skilled Medical Therapist to join our team as a Patient Care Specialist. In this role, you will work collaboratively with our Physical Therapists to provide exceptional patient care and support.

About the Role

This is a full-time position based in Augusta, Maine. As a Patient Care Specialist, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

Responsibilities
  • Implement physical therapy services in accordance with the plan of treatment developed by the PT and signed by the physician, under planned, delegated, and supervised guidance from the RPT.
  • Collaborate with the RPT to measure the client's functional level by applying appropriate tests and reporting results to the PT.
  • Implement client care plan activities based on treatment goals towards independent living for the client/family as directed by the Physical Therapist.
  • Provide clear direction to clients during visits and communicate goals for the next visit.
  • Observe and document the effectiveness of interventions on the physical therapy visit report and report to the PT.
  • Confer with the RPT for referrals to internal and external community resources to meet client outcomes when appropriate.
  • Treat clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.
  • Teach and supervise the client and family regarding alternative techniques to complete activities of daily living, proper transfers, and positioning.
  • Instruct and advise clients, family, and other health team personnel, including home health aides, in certain phases of physical therapy where they may assist the client; also instruct all of the above in the use of prosthetic and orthotic devices and durable medical equipment if appropriate.
  • Identify team members and their roles in case management activity and work collaboratively with them.
Requirements

To be successful in this role, you will need:

  • A Graduate degree from an accredited Associate Degree Program for Physical Therapy Assistants.
  • A minimum of one year recent clinical experience.
  • Current licensure in the State of Maine as a Physical Therapy Assistant.
  • A reliable automobile with a current Maine driver's license.
Estimated Salary Range

$60,000 - $80,000 per year, depending on experience.
