Job Title: Advanced Practice Provider

Join the Medical College of Wisconsin's Department of Surgery, Division of Trauma and Acute Care as an Advanced Practice Provider in Menomonee Falls, WI at Community Memorial Hospital.

About the Role:

The Advanced Practice Provider will work as a member of a multidisciplinary healthcare team, contributing to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

Key Responsibilities:
  • Collaborate with interdisciplinary healthcare teams in diagnosis and treatment of conditions requiring surgical intervention in an inpatient environment.
  • Write accurate and timely progress notes, procedure notes, and/or admission and transfer orders.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicate and document information.
  • Privileged to perform procedures beyond core as identified by the acute care surgery services.
  • Facilitate and communicate with patient, family, and staff to promote continuity of care across the continuum of care.
  • Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission.
  • Evaluate quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
  • Acquire information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature.
  • Attend daily rounds with attendings and Acute Care Surgery physician and APP team, presents 24-hour patient report.
  • Act as a liaison for communication of care plans and discharge destinations from ICU to floor teams. Attend care coordination rounds as able.
  • Participate in Acute Care Surgery performance improvement activities to ensure the quality of care and effectiveness of service.
  • Complete complex wound care and negative pressure wound therapy.
  • Provide education and facilitates communication between surgical residents and medical students as required.
  • Assistance in the operating room as needed.
  • Complete consultations as required from referring teams.
Core Advanced Practice Responsibilities:
  • Perform comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive, and psychosocial status.
  • Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
  • Write accurate and timely progress notes, procedure notes, and/or admission and transfer orders.
  • Develop pharmaceutical and non-pharmaceutical treatment plans.
  • Monitor effectiveness of plan of care.
  • Educate patients on disease and treatment plans.
  • Perform procedures as trained and appropriately credentialed.
  • Document within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborate and/or consults with physicians and other healthcare providers as appropriate.
  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.
  • Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Joint Commission.
  • Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature.
Requirements:
  • Advanced Practice Nurse: Master's Degree or higher in Nursing
  • Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies
  • Wisconsin RN License
  • Wisconsin Advanced Practice Nurse Prescriber License
  • Certification by NCCPA
  • AHA ACLS, AHA BLS, DEA
  • Acute Care preferred
