Outpatient Geriatric Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our multidisciplinary therapy team in the Marietta area. As a Physical Therapist with Career Tree Network, you will have the opportunity to work with a well-balanced mix of patients in their homes, providing community-based outpatient services under Medicare Part B.
Job SummaryThis is a full-time, part-time, or PRN position that includes flexible hours based on your preference. At a minimum, we are looking for someone to see 12.5 hours of direct patient care per week. The successful candidate will be responsible for performing outpatient therapy within home-based settings, working closely with geriatric patients and their families to develop personalized treatment plans.
About the RoleThe Physical Therapist hired for this role will be an integral part of our team, working collaboratively with other healthcare professionals to provide high-quality care to our patients. As a member of our team, you will have access to ongoing training and support, ensuring you stay up-to-date with the latest best practices and evidence-based care methods.
Responsibilities- Providing individualized physical therapy services to patients in their homes, focusing on geriatric rehabilitation and wellness
- Developing and implementing comprehensive treatment plans tailored to each patient's unique needs and goals
- Collaborating with other healthcare professionals to ensure seamless care coordination and communication
- Maintaining accurate records and documentation, adhering to regulatory requirements and professional standards
To be considered for this role, you must possess:
- Graduation as a Doctor of Physical Therapy (or equivalent standing)
- Eligibility for Georgia state licensure as a Physical Therapist
- A minimum of 12.5 hours per week of direct patient care availability
- Regular and consistent weekly availability, including three weekdays
- Professionalism, accuracy, and dependability
- Possession of passion and enthusiasm for providing creative evidence-based care
New graduates will receive support and mentorship for part-time availability or higher, while candidates interested in PRN/per diem positions will require prior work experience. If selected for PRN/per diem, ongoing one-on-one direct patient care will be performed.
