As a key member of our team, you will be responsible for providing nursing care management services to assigned populations.

You will utilize a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.

In collaboration with physicians, you will identify at-risk patient populations who require care management services.

You will serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.

You will understand the needs of the organization and support the mission, values, and management of patient care services.

You will actively support and incorporate the ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.


Requirements:
8 Years Nursing-Related Area Required

Education:
Bachelor's Degree Nursing Required

Licensure/Certification:
Registered Nurse Required

Work Schedule:
M-F

Weekend Requirements:
No Weekends
