Job Summary

Maryland Wellness is seeking a highly skilled Rehabilitation Counselor to join our Psychiatric Rehabilitation Program (PRP) team in Baltimore. As a key member of our team, you will be responsible for providing direct support services to clients in their homes and/or the community, facilitating their journey towards recovery and improving their functioning and quality of life.

Key Responsibilities
  • Conduct regular in-home or community-based client visits to address client goals and provide individualized case management, coaching, and skills training as needed.
  • Develop and implement Individual Rehabilitation Plans (IRPs) with clients to outline goals for recovery.
  • Identify and connect clients with government and/or community resources as needed.
  • Support client cooperation with treatment recommendations from other providers and maintain communication with those providers.
  • Provide and/or coordinate transportation to and from scheduled appointments.
  • Ensure timely and accurate completion of all documentation, including notes, monthly progress summaries, and quarterly IRP reviews.

  • Must be highly flexible and able to adapt to change.
  • Self-disciplined, self-starter, and able to work independently with a flexible work schedule.
  • Outstanding interpersonal skills with the ability to effectively build relationships with individuals of all ages, ethnicities, education levels, and work experience.
  • Must be able to operate a vehicle for travel expectations within the community.
  • Possesses the ability to strategically, logically, and realistically analyze, evaluate, and develop solutions for a client's rehabilitation needs.
  • Understands and maintains confidentiality and all regulations regarding HIPAA and 42 CFR Part 2.
  • Exhibits professionalism, a friendly demeanor, and is a positive representative of the company over the phone and in person.
  • Excellent written and verbal communication skills.
  • Comfort with computers and technology as needed to input clinical documentation.
  • Strong multi-tasking and organizational skills to manage multiple clients and scheduling.
